You need a Google account in order to access Forms within Google Documents.
Forms are used to create surverys/questionnaires that you can either send to a list of people or make public on the Internet. What is nice about Forms is that you receive immediate feedback from responses as they are collected automatically in a spreadsheet. You can also receive email updates as responses are submitted from your Form.
